How is it piped. A zone off your boiler with a circulator ?

1" copper, 1" taco zone valve, wired to Taco zone vale controller, hwh zone valve on the priority circuit. Hope this helps

Did it used to work OK or has it been like this from day one?
Is the temp good when it first starts to provide hot water?

Almost sounds like the hot and cold supply are reversed
OR
the cold supply pipe inside the tank is broken and cold water is not entering at the bottom of the tank

Next time it goes cold try draining some water out, if it is hot then cold water is not getting to the bottom of the tank. Try to do this as soon as it gets cold or is just warm so the cold water does not have time to drop to the bottom.
